Creating and Managing Surveys

How surveys differ from forms, when to use them, and how to build multi-page questionnaires for intakes, feedback, and discovery calls.

Creating and Managing Surveys

Surveys are like forms — but with superpowers. The big difference? Surveys can have multiple pages, letting you break longer questionnaires into manageable steps. They're perfect for anything that needs more than a few quick fields.

Forms vs surveys — when to use which

The editing tools are almost identical (if you're comfortable with creating forms, you already know how surveys work). The key differences are: Watch this step (0:00)

Use a form when you need something quick — name, email, a short message. One page, done.

Use a survey when you need to collect more information and want to break it into steps so it doesn't overwhelm people.

When to use surveys for your wellness business

  • Client intake before a coaching session — page 1: personal info, page 2: health history, page 3: goals and expectations
  • Retreat application — page 1: contact details, page 2: experience level and preferences, page 3: dietary needs and payment
  • Post-retreat feedback — page 1: overall rating, page 2: what they loved, page 3: suggestions for improvement
  • Discovery call questionnaire — page 1: about their business, page 2: what they're looking for, page 3: budget and timeline

Building a survey

Navigate to Sites → Surveys to see your surveys. Watch this step (0:32)

The editor works the same way as the form builder — drag fields, edit on the right panel, set up conditional logic. Watch this step (0:43)

If you haven't read the Creating Forms article yet, start there. It covers the builder in detail — adding fields, editing options, conditional logic, styling, and notifications. Everything in that guide applies to surveys too.

The big difference: multiple pages

This is what makes surveys special. Each page can serve a different purpose. Watch this step (1:05)

For example, a retreat application survey might look like:

Page 1: About you

Name, email, phone, where they're based.

Page 2: Your experience

"Have you attended a retreat before?", experience level, any health considerations, dietary requirements.

Page 3: Payment

Select a retreat package, enter payment details, accept terms and conditions.

Each page has a "Next" button so people move through step by step. This feels less overwhelming than one giant form, and completion rates are typically higher because people commit one step at a time.

You can also use conditional logic between pages. For example, if someone selects "First-time retreater" on page 2, you could show them an extra page with beginner-friendly information before the payment page.

Sharing your survey

Click Integrate to get your sharing options. Watch this step (1:57)

Surveys have two sharing options:

  • Embed code — add it to your website (or ask Korneel to embed it for you)
  • Share link — a direct URL you can send via email, WhatsApp, or social media

Unlike forms, surveys cannot be embedded directly in emails. If you want to send a survey to someone, share the link in your email instead. The recipient clicks the link and opens the survey in their browser.

After someone completes the survey

Just like forms, you can set up actions that happen automatically after submission. Watch this step (2:10)

  • Show a thank-you message
  • Redirect to another page
  • Trigger an automation (like a follow-up email or booking confirmation)
  • Create a contact in your system (happens automatically)

All submissions are also visible in the Submissions tab, just like forms. Learn about viewing submissions
